Do not complete this form if you have not first attempted to resolve the hold(s) on your student account.

For mandatory advising holds, contact your Academic Advisor to schedule a meeting. Once you have met and satisfied the reason for the hold, the hold will be lifted and you will be able to withdraw via Banner.

For all other holds, visit the student holds website to learn how to resolve this matter. Once you have satisfied the reason for the hold, the hold will be lifted and you will be able to withdraw from via Banner.

If you are unable to meet with your academic advisor or resolve your hold prior to the withdrawal deadline, please complete the form below, and we will work to process your request.

Will I get a W or WF?

Students who request a withdrawal before the deadline will receive a grade of W, which does not have any impact on a student’s GPA. Withdrawal requests submitted after the withdrawal deadline will result in a WF (withdrawal fail), which will negatively impact a student’s GPA.

To avoid academic penalty (a WF grade), please submit your form prior to the withdrawal deadline, as noted on the Academic Calendars listed below:

Hardship Withdrawal Requests

If you are experiencing a situation that makes it impossible to continue in your course(s) including but not limited to: mental health concerns, physical health concerns, or a family emergency and the withdrawal deadline has passed, you may request a hardship withdrawal through the Dean of Students office. Documentation will be required to verify these circumstances.

Your request will be processed by our withdrawal team according to the date that this form was submitted.

I acknowledge upon a complete withdrawal from the university (through the Registrar’s Office), a partial refund of tuition and fees will be processed. A complete withdrawal is a withdrawal from all courses. Individual course withdrawals will not generate a refund.
